Focus On: Medical Record Coding

Results-Oriented Approach to Medical Record Coding

Using a well-managed approach to medical record coding and auditing, HCRS delivers:

  • Standardized and reliable data for reporting purposes;
  • Improved coding compliance, coding accuracy and data quality;
  • Increased reimbursement to clients;
  • Enhanced front-end and back-end revenue cycle processes leading to improved efficiencies; and
  • Improved relations among and between health information management experts, billing staff, providers, clinic staff, system experts, and data quality managers involved in the Revenue Cycle process.

We set the following production standards for our medical records coders:

  • Outpatient: 15 records per eight (8) hours, or 120 records per individual per day
  • Inpatient: 2-3 records per eight (8) hours, or 16-24 records per individual per day
  • Coding accuracy rate of at least 95 percent

In addition, HCRS has developed and delivered training courses for coders including:

  • Training to providers, clinical staff and coders on topics including Documentation and Coding Guidelines; Basic ICD-9; CPT and E&M Coding;
  • Advanced courses on ICD-9 Documentation and Coding, Coding E&M Elements, and Keys to Using Modifiers;
  • ICD-9 and ICD-10 conversion implementation and training; and
  • Clinic-specific training on topics related to documentation and coding for medical services such as Flight Medicine, Neurology, Orthopedic, Physical Therapy, Pulmonary and OB-GYN.

HCRS’ E-coder Software

HCRS has developed a Structure Query Language (SQL) database to automate the manual process of capturing coding information.

Our Electronic Daily Coding Productivity Log (EDCPL) improves efficiencies by replacing labor-intensive manual processes with an automated Web-based process – significantly reducing lost man-hours while also improving data integrity, recording, reporting and maintenance for a coder’s daily productivity information. In addition, coders and program managers can use EDCPL to identify any issues and implement corrective action before they have an effect on the contract performance.
